Complete and return this form to the advisor before your child's first session. All fields are required. Participation is voluntary and may be withdrawn at any time.

About the program

VIBE Afterschool is a free, extracurricular program for students in grades 7–12. One afternoon a week for twelve weeks, students work with an advisor (a teacher, counselor, or vetted community volunteer with AI experience) to build a real project using AI tools. AI-fluent guides from the community visit when available. A school staff member is present at every session per school policy. The semester ends with a public showcase where students present their work. There is no cost to students or families.

I give permission for my child to

Participate in the program. Attend weekly sessions during the semester listed above and complete a project for the end-of-semester showcase.

Use AI tools during sessions. Tools are advisor-supervised and approved by the host before the cohort begins. Students typically use their own internet-connected device (phone, tablet, Chromebook, or laptop) during sessions and at home; some hosts also provide devices for in-room use. Student work is not submitted to AI platforms for training. The program’s AI Acceptable Use Policy applies.

Interact with volunteer professionals. Any community professionals invited as visiting guides will visit sessions when available. Every adult in the room has passed the school’s standard background check. A school staff member is present for every session per school policy. No one-on-one contact between guides and students occurs, during or after sessions.

Participate in the public showcase. At the end of the semester, students present their projects to an audience that may include families, school staff, sponsors, and local press.

Photo and video (check one)

YES. Sessions and the showcase may be photographed or recorded. Images or video may be used for VIBE Afterschool program documentation, the school’s communications, and program outreach. My child will not be identified by full name in public materials without separate permission.

NO. My child should not be photographed or recorded. The school will inform the advisor.

Student data and privacy. VIBE Afterschool does not collect, store, or transmit individual student data. Program outcomes are reported in aggregate only. The school’s existing data privacy agreements with technology vendors govern tool use. Student education records remain the school’s responsibility under FERPA. All students must be 13 or older to participate.
